Session Rates
Appointments rates are as followed:
Samantha Earley, MS, LPC, & Owner: $150 per 50-minute counseling session (individual, couples)
LPC-Associates rates: $120 per 50-minute counseling session (individual, couples)
Telehealth sessions are billed at the same rate as in-person
Payment for sessions is due at the time of the appointment. All payments are processed through an online HIPAA-compliant software, and credit card are kept on file
Sliding Scale rates may be available, check with practice for more information.
I am not contracted with any insurance companies at this time. You may file claims with your insurance company for out-of-network benefits, and you may be reimbursed directly based on your insurance plan. A receipt and superbill can be provided upon request.
Please give at least 24-hour notice if you need to cancel or reschedule an appointment for any reason. If you do not cancel before 24 hours, the full session rate with be charged to your card (late cancelation) or a no show. You are responsible for calling to cancel or reschedule your appointment in a timely manner during regular business hours.

Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost. Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
Under Section 2799B-6 of the Public Health Service Act, the “No Surprises Act,” you have a right to receive a “good faith estimate” explaining how much your therapy services will cost.
• You have the right to receive a Good Faith Estimate for the total cost of therapy services
• If you receive a bill that is at least $400 more than your Good Faith Estimate you can dispute the bill.
